Champ Health app for iPhone and iPad

This project is developed a CHAMP (Canadian Chronic Disease Awareness and Management Program) IPhone/IPad application to manage cardiovascular diseases using protocols defined by CHAP (Cardiovascular Health Awareness Program). Since 2000 in Ontario - Canada, there is an innovative program designed to manage cardiovascular health awareness (CHAP). The main concept is “ a community-based program that brings together local family physicians, pharmacists, volunteers, public health representatives & other organizations to work as partners to promote and actively participate in the prevention and control of cardiovascular disease”.
